I have read the booklet. While it provides good insight, it is limited in scope. I find the format different from the traditional 3 column approach normally used. That format allows for a better flow and view of the steps for a job or task. Neither format clearly addresses risk at each level. The issue is not so much the form but the data gathering and evaluation in order to get a clear picture of what is actually being done. Having a full JHA process can lead to better results.
