This RN position will work as a member of the Rehab. Team.
The Clinical Nurse II - Home Care Nurse practices in the role of primary and/or associate nurse within the professional practice model and demonstrates core values with reflect the hospitals mission. The nurse assumes responsibility for the plan of care and utilizes the Nursing process in the delivery of care. The nurse facilitates coordination of care across the continuum. The staff nurse demonstrates safe practice within the domains of clinical judgement and decision making, nurse-patient/family relationship, clinical leadership and clinical scholarship. The nurse maintains a safe level of knowledge and skill with technical procedures and interpersonal communication and also demonstrates the knowledge and skill necessary to provide the expected standards of care.
Graduate of an accredited school of nursing; BSN required. Currently licensed as a registered nurse in the State of Pennsylvania. Must have current Pennsylvania Driver's license. One year of full-time experience in nursing is required; preferably in home health setting.
